What you'll do

• Assist with resident and family interviews and psychosocial assessments
• support adjustment to facility placement
• help determine legal status and obtain advance directives
• inform residents and families of rights
• coordinate evaluations and appointments
• connect residents with financial, legal, mental health, and community resources
• support documentation, resident rights concerns, and discharge planning.
