What you'll do

• Remain available by telephone and email during assigned shifts
• conduct urgent physical and psychosocial assessments after hours
• determine appropriate interventions
• provide nursing care and pain and symptom management
• guide patients and families on diagnoses, medications, treatments, and progress
• document care and clinical changes
• communicate with physicians and obtain orders as required
• collaborate with the interdisciplinary group, vendors, hospitals, and community residential staff
• conduct after-hours hospice admissions as necessary.
