What you'll do

• Educate and counsel on nutrition, exercise, stress, and chronic disease management
• assess nutritional status and document findings
• develop and revise nutrition and chronic disease care plans
• support diabetes and obesity management, including inpatient diabetes interventions and self-management education
• assist with program accreditation and required training
• participate in community relations events, health fairs, and presentations
• maintain knowledge of program competencies.

Why this role
