Rapid Rehousing Case Manager

The Rapid Rehousing Case Manager role is onsite in Fort Worth, TX. Compensation is not specified. The case manager provides intensive, trauma-informed support to help families find and maintain permanent housing and reduce returns to homelessness. The role coordinates housing placement, service planning, community referrals, and ongoing assessment with participants and the multidisciplinary team.

What you'll do

• Support housing searches and placement
• build relationships with landlords, housing authorities, and community agencies
• complete housing quality inspections and lead-based paint assessments
• coordinate move-ins and housing documentation
• reassess rental assistance eligibility
• conduct trauma-informed assessments and service planning
• use motivational interviewing
• develop safety plans
• participate in multidisciplinary team meetings
• connect families with supportive services
• address conflicts and document incidents as needed.

Why this role
