Registered Nurse (RN) Case Manager — PACE

What you'll do

• Manage an assigned participant caseload and serve as the primary nursing contact
• track clinical status, care needs, medications, supplies, and follow-ups
• conduct home, clinic, and facility visits
• complete assessments and update care plans
• monitor chronic conditions and educate participants and caregivers
• collaborate with the interdisciplinary team
• coordinate transitions, referrals, and discharge follow-up
• participate in an on-call rotation
• maintain clinical records and regulatory compliance
• support quality improvement and staff learning.
