Licensed Clinical Social Worker (LCSW)

Completes social work/psychosocial assessments on newly admitted patients. Reviews the assessments completed by the other inpatient social workers. Ensures goals and an individual plan of care are established for patients by the inpatient social workers. Collaborates with individuals, families, health system and community based interdisciplinary professionals on the plan of care.
