What you'll do

• Oversee handling of inbound referrals and support after-hours coverage when needed
• coordinate outbound referrals when the center cannot admit a patient
• liaise with referred patients and external partners
• track stepdowns from detox and residential programs
• coordinate transitions and documentation in Salesforce
• report referral activity and trends
• maintain CRM data accuracy
• identify pipeline issues and communicate outcomes to leadership.
