The Case Manager – Ivy Senior Permanent Supportive Housing role is an onsite, full-time position in San Diego, CA. Pay is $23–$28 per hour. PATH provides homelessness prevention, outreach, case management, supportive services, interim housing, and permanent supportive housing across California. The case manager helps participants address housing instability and pursue personal goals by assessing needs, developing individualized service plans, and connecting clients with healthcare, social services, and other resources. The role supports clients through different phases of homelessness and helps them access and maintain services.

What you'll do

• Conduct participant intake and individualized needs assessments
• collaborate on service plans addressing housing, income, and personal goals
• provide referrals, linkages, information, and support
• complete program-specific assessments
• evaluate strengths and challenges
• conduct a 90-day review after initial intake. Driving is an essential function.
