What you'll do

• Perform case management duties at an advanced level
• analyze length of stay, costs, outliers, and potential discharge-planning issues
• assign patient caseloads to support delivery of case management services
• lead interdisciplinary collaboration with departments, medical staff, and hospital leadership
• precept and assist with onboarding, orientation, and training of new staff or students
• maintain knowledge of performance improvement programs, CMS Conditions of Participation, Medicare Coverage Criteria, and relevant Joint Commission standards
• oversee department staff and operations in the absence of the DCM.
