Patient Navigator

Job overview

The Patient Navigator role is onsite in Kissimmee, FL. Pay is $16–$24 per hour. PeopleOne Health focuses on preventive, value-based primary care intended to improve health outcomes and reduce costs. The navigator helps patients access specialty care and other health services by coordinating referrals, addressing barriers, explaining next steps, and maintaining communication among patients, clinical staff, providers, and external organizations.

What you'll do

• Coordinate and process referrals to specialty providers, imaging services, and other healthcare resources
• communicate with referring and receiving providers
• track referrals and follow up to support completion
• educate patients about insurance requirements, appointment preparation, and follow-up
• assist with scheduling, transportation, language services, and other needs
• document referral activity and patient interactions in the electronic medical record
• participate in community outreach and care-coordination initiatives.
