What you'll do

• Assess psychosocial risk factors and resources relevant to discharge
• collaborate with patients, families, physicians, and care teams on care progression and discharge plans
• identify delays and barriers and coordinate resolution
• manage discharge planning and post-acute referrals
• participate in interdisciplinary rounds
• document assessments, interactions, and arranged services
• track avoidable days
• assist with precepting and mentoring when needed.
