The Medical Assistant (Health Navigator), Senior Health role is onsite in Glendale, CO, with a Monday–Friday schedule from 8:00 a.m. to 5:00 p.m. Compensation is $23.00 per hour based on a full-time schedule. One Medical’s primary care model focuses on making quality care more affordable, accessible, and enjoyable; this office serves a Senior Health member population age 65 and older. The Health Navigator supports a primary care team and its patient panel by gathering health information, coordinating follow-up care, and helping patients navigate preventive, chronic, and acute care needs. The role contributes to a supportive in-office experience and better care coordination through collaboration with providers, patients, and families.

What you'll do

• Room patients, take vital signs, administer point-of-care testing, and perform age- and condition-appropriate screening assessments
• schedule specialist appointments and coordinate referrals
• help providers monitor preventive care
• support practice operations, including calls, emails, front desk inquiries, and clinical space upkeep
• assist with patient education and chronic disease action planning.
