What you'll do

• Coordinate assessments and maintain required documentation
• participate in developing, implementing, and reviewing Individual Support Plans

• monitor progress and communicate concerns
• liaise with families, providers, employers, and community agencies
• guide staff on individual needs and plan implementation
• support community activities and medical appointments
• assist with regulatory compliance and quality assurance.
