What you'll do

• Increase use of preventive care and needed mental health services
• assess unmet social needs
• develop intervention plans with clients, families, and providers under mental health care-manager supervision
• connect clients with community resources and assistance programs
• conduct school and community visits
• monitor intervention plans and progress
• participate in care and wellness meetings
• document client interactions and resources
• share concerns with the care team
• attend mandatory training
• maintain confidentiality and follow HIPAA guidelines.
