What you'll do

• Participate in interdisciplinary and comprehensive patient assessments
• contribute to and evaluate plans of care with patients, representatives, and caregivers
• provide physician-ordered services
• educate patients and families
• initiate preventive and rehabilitative nursing procedures
• complete timely clinical notes
• monitor outcomes and communicate changes
• supervise paraprofessional staff and assign home care aides
• coordinate discharge planning and care with the healthcare team
• participate in quality improvement, training, and emergency preparedness as requested.
