Registered Nurse/Licensed Practical Nurse (RN/LPN) - Part-Time Evening Shift, onsite in Plaistow, NH. This part-time position offers a pay rate starting at $39.50+/hr, with a $3000 sign-on bonus. This role is responsible for providing direct resident care in a team-focused environment within the Skilled Nursing unit. The RN/LPN will collaborate with the interdisciplinary team to ensure comprehensive and individualized care plans are maintained and executed.

What you'll do

• Performs professional nursing duties in accordance with facility policies, State Nurse Practice Act, OBRA, and HIPAA.
• Collaborates with the interdisciplinary team to formulate and update resident care plans.
• Institutes care protocols for falls prevention, safety, behavior management, skin integrity, etc.
• Assesses and documents changes in resident status, reporting to physicians.
• Provides leadership and direction to caregivers, delegates workload, and supervises direct care.
• Obtains, administers, and documents medications and treatments.
• Maintains positive working relationships with team members, residents, and families.
• Supports a positive culture for team members and residents.

What we're looking for

Education

Nursing Degree or Diploma

Experience

Minimum of a Nursing Degree or Diploma from an accredited institution. Long-term care facility experience and knowledge of geriatric/rehabilitative nursing preferred.
