Hospice Social Worker

What you'll do

• Conduct psychosocial assessments within five days of hospice admission
• participate in interdisciplinary group meetings and care planning
• document evaluations, care plans, and notes
• assess patient and family responses and coping
• coordinate visit schedules with case managers
• support discharge planning and connect patients with family and community resources.
