What you'll do

• Evaluate patients and physician orders for home care eligibility
• conduct face-to-face transitions and provide agency education
• document encounters
• coordinate transfer orders and ancillary services such as DME and infusion
• communicate with referral sources and healthcare providers
• support outreach and admission goals
• educate patients on post-discharge appointments, prescriptions, and medication regimens.
