The Pharmacist role is onsite in New Albany, IN; compensation is $114,400–$156,000 per year. KabaFusion is a home infusion company with a national network of pharmacies and nursing offices serving more than 40 states. The role exists to provide safe, appropriate pharmacy services to patients in alternate care settings. The pharmacist assesses therapy, dispenses medications and supplies, coordinates patient services, documents progress, educates patients and caregivers, and collaborates with the care team to support effective home infusion treatment.

What you'll do

• Assess patient suitability for home care and prescribed therapy
• develop patient-specific pharmacy plans of care
• recommend and write appropriate orders
• assess laboratory results and perform pharmacokinetic dosing
• maintain medication profiles and enter prescription orders
• dispense medications and supplies
• troubleshoot medication administration pumps
• document communications and patient progress
• ensure compounding activities meet applicable standards
• participate in patient care conferences
• participate in the on-call process
• oversee and train pharmacy support personnel.
