What you'll do

• Provide skilled nursing care according to patient care plans
• assess physical, emotional, and psychosocial needs
• coordinate with the interdisciplinary team, patients, and families
• provide education and support
• administer prescribed medications and treatments
• and document care and changes in patient status. Triage calls and make visits as necessary during the every-other-weekend coverage period, Friday at 4:30 p.m. through Monday at 8:00 a.m.
