What you'll do

• Assist intake therapists with referrals and eligibility information
• assess non-clinical barriers and coordinate resources
• facilitate referrals to internal programs and community services
• monitor engagement and document contacts
• help families complete forms and navigate service systems
• participate in multidisciplinary meetings
• maintain confidential records
• support outreach and community education.
