Case Manager — Inpatient Psychosocial Care

What you'll do

• Assess psychosocial needs and treatment needs
• counsel patients and families
• make referrals to home health and community agencies
• coordinate and revise discharge plans
• complete intake assessments, psychosocial histories, and treatment plans
• help determine eligibility for services
• communicate with the multidisciplinary team and physicians
• conduct individual and group therapy
• implement crisis intervention when indicated
• participate in program planning, quality improvement, safety, and infection-control studies as assigned.
