What you'll do

• Participate in interdisciplinary care planning
• evaluate, diagnose, and manage participant medical conditions
• conduct histories and examinations, order and interpret diagnostic tests, develop treatment plans, prescribe, and follow up
• document care
• manage participants in contracted nursing facilities
• supervise assigned staff
• support quality, utilization, compliance, and primary-care initiatives
• supervise the after-hours coverage plan
• travel to participant homes and other off-site locations.
