What you'll do

• Assess, plan, implement, and evaluate individualized home-care plans
• administer ordered medications and monitor for adverse reactions
• monitor and document symptoms, condition changes, and vital signs
• coordinate care and tests with providers, families, and departments
• educate patients and families
• maintain records and follow safety, infection-control, confidentiality, and compliance standards.
