What you'll do

• Assess spiritual needs of hospice clients and families/caregivers and ensure timely services
• participate in individualized care planning
• provide direct spiritual care
• liaise with community chaplains and spiritual counselors
• maintain service records
• consult with and educate the interdisciplinary group
• provide requested funeral or memorial services and plan periodic memorial services
• drive between client homes.
