Patient Navigator is an onsite, full-time role in Miami Gardens, FL. Pay is $20 per hour. The position supports patient engagement and access to care at the Center for Family and Child Enrichment’s Federally Qualified Health Center, which provides services and solutions for children and families. The role helps patients understand their assignment to the health center, learn about available services, and navigate onboarding and access barriers. By coordinating outreach, referrals, and appointments with clinical and front-office teams, the navigator supports timely connections to appropriate care.

What you'll do

• Call patients on capitation rosters to explain their assignment to the health center and educate them about primary, dental, behavioral health, and specialty services. Guide new patients through onboarding and direct them to departments for scheduling. Follow up on Sliding Fee Discount Program eligibility and applications
• identify access barriers and connect patients with resources. Screen for social needs, coordinate appointments and referrals, respond to inquiries, and document outreach and interactions in EHR and call-tracking systems while maintaining HIPAA confidentiality.
