Clinical Care Coordinator

Job overview

The Clinical Care Coordinator role is onsite in Harlem and Jamaica, NY; compensation is $70,000 per year. Elmcor’s shelter-based social services team supports families residing in the shelter, and the position reports to the Director of Social Services. The coordinator provides supportive counseling and clinical guidance to families, including those facing behavioral health, substance use, or intimate partner violence challenges. Through assessments, crisis intervention, referrals, and coordinated care, the role supports family participation and access to appropriate services.

What you'll do

• Conduct biopsychosocial assessments and clinical screenings
• conduct psych-evaluations for 2010E housing applications using DSM-5 diagnostic criteria
• support high-need families using trauma-informed care
• use motivational interviewing and harm reduction principles
• provide crisis intervention and refer clients to the Housing Specialist
• coordinate care with facility staff and providers
• maintain contact with families and support program participation
• provide clinical guidance to the social service team
• create and facilitate staff training
• attend meetings and case conferences
• make referrals and advocate for clients.
