Hospice Chaplain

The Hospice Chaplain works onsite in Cleveland, OH; compensation is not specified. What you'll do

• Provide or coordinate spiritual care for patients, families, and caregivers
• conduct spiritual assessments
• assist with bereavement follow-up in collaboration with the bereavement coordinator
• participate in care conferences, hospice admissions, and Interdisciplinary Group meetings
• support hospice team members
• maintain relationships with local spiritual leaders
• participate in quality improvement
• participate in the designated weekend on-call rotation.

Why this role

Requires ecclesiastical endorsement and at least two units of Clinical Pastoral Education from an accredited ACPE center. A Master of Divinity degree or equivalent is preferred.
