Through clinical assessments, timely communication, and coordination with admissions and interdisciplinary teams, the liaison helps residents and families understand post-acute care options and supports successful transitions.

What you'll do

• Develop relationships with hospitals, physicians, case managers, social workers, discharge planners, and other referral sources
• conduct hospital rounds
• identify and respond to referrals
• coordinate with admissions and intake
• complete clinical assessments and bedside evaluations when appropriate
• communicate admission updates
• follow referrals through admissions
• maintain CRM/EHR activity
• participate in provider meetings, presentations, and community outreach.
