What you'll do

• Plan, organize, implement, and evaluate the social services program
• maintain policies and documentation
• prepare for surveys and inspections
• complete psychosocial assessments, social histories, and applicable MDS sections
• participate in care planning
• provide therapeutic support for transitions
• educate residents and families about rights and resources
• coordinate discharge planning and referrals
• collaborate with staff and community agencies.
