The physician provides comprehensive primary care across the lifespan, with emphasis on prevention, chronic disease management, and whole-person health. Working with an interdisciplinary care team, the role supports continuity of care and quality goals for a diverse and often underserved patient population.

What you'll do

• Provide diagnosis, treatment, and management of acute and chronic conditions for patients of all ages and genders
• deliver annual exams, immunizations, screenings, and health education
• manage chronic diseases using evidence-based guidelines
• perform routine in-office procedures
• provide women’s health services including contraceptive counseling
• coordinate with behavioral health, dental, and specialty providers
• document encounters in the EHR
• participate in care team huddles, case conferences, and quality improvement
• support population health outreach and care gap closure
• provide culturally competent, trauma-informed care
• participate in after-hours call rotation as needed.
