The role supports intake, assessments, referrals, follow-up, required records, and program reporting in response to youth and program needs.

What you'll do

• Assist with intake, assessments, case management, and paperwork
• oversee educational referrals
• answer crisis and information/referral calls
• provide individual and family case management and help access community resources and housing assistance
• monitor client care files
• coordinate aftercare and follow-ups
• support program implementation, reporting, and data entry
• participate in training and community networks.
