What you'll do

• Coordinate services and benefits for individuals admitted to the crisis stabilization program
• track contract metrics and program outcomes
• provide direct clinical services and periodic substance use assessments
• help clients access resources, benefits, and entitlements
• support treatment-plan goals
• collaborate with community partners and primary and dental care providers
• assist with Personal Recovery Plans under clinician direction
• provide transportation when appropriate
• participate in home visits and welfare checks when appropriate
• maintain timely clinical documentation
• coordinate BestClub services
• participate in training and help orient new staff.

Why this role

Level I qualification requires an associate degree in behavioral science plus four years of demonstrated social services or counseling work. Requires experience with people with severe and persistent mental illness and QMHA-I certification or eligibility to register for QMHA-R and obtain certification within six months.
