What you'll do

• Provide companionship
• assist with meal preparation, light housekeeping, errands, bathing, dressing, grooming, transfers, and ambulation
• accompany clients to appointments or social outings and provide transportation as needed
• monitor and report changes in client health, behavior, and needs
• communicate with the RN Case Manager about care plans.
