Hospice Chaplain

This role exists to assess and address spiritual needs as part of individualized hospice care. The chaplain provides direct support, coordinates with community spiritual counselors, and collaborates with the interdisciplinary team to promote continuity of care.

What you'll do

• Assess spiritual needs of patients and families/caregivers and ensure timely services
• provide direct spiritual care
• contribute to individualized care plans and interdisciplinary meetings
• coordinate with community chaplains and spiritual counselors
• document services and maintain records
• educate and support the interdisciplinary group
• recruit community spiritual care volunteers
• provide requested funeral or memorial services
• participate in on-call duties.

Why this role

Master’s degree in theology, divinity, or pastoral studies required; certification preferred. Hospice experience preferred; interfaith comfort and ongoing Clinical Pastoral Education development expected.
