By monitoring needs and following through on service plans, the role helps individuals receive coordinated support and may help prevent escalation or the need for restrictive acute care.

What you'll do

• Partner with treatment teams and natural supports to develop resiliency plans and evaluations.
• Link and coordinate services, follow up with agencies to confirm resources are in place, and share client updates with interagency teams.
• Maintain required client documentation and monitor services through check-ins and community resource connections.
• Perform CPR and agency-trained restrictive procedures and manual restraints.

Why this role

Bachelor’s degree or above in behavioral health or a related human service field required. Current valid driver’s license and acceptable driving record required. Behavioral health experience and EHR/EMR knowledge preferred.
