Enhanced Care Management Lead

The Enhanced Care Management Lead role is onsite in Escondido, CA, with compensation of $27.21–$38.09 per hour. Neighborhood Healthcare’s ECM program supports high-utilizer and vulnerable patient populations. The lead supports daily care-management operations and helps the program meet its goals by training and mentoring staff, coordinating care plans and community services, and providing direct care management to assigned participants.

What you'll do

• Train and mentor staff
• support daily ECM operations and production goals
• coordinate with leadership, providers, managed care teams, and community organizations
• develop care and service plans
• assist with staffing schedules and partner coordination
• review case files and provide feedback
• conduct outreach, enrollment, assessments, and reporting
• provide direct care management and help members navigate healthcare
• document services and support quality improvement.
