What you'll do

• Conduct telephonic or face-to-face assessments within licensure scope
• collect clinical data and help identify members at risk or who may benefit from another level of care or waiver program
• provide collected information to the responsible RN
• participate in care coordination for members with chronic illnesses, co-morbidities, or disabilities as directed by the RN
• support efficient use of health benefits.
