Non-Clinical Care Navigator – Oncology

This work supports access to care and communication across the treatment team and care continuum.

What you'll do

• Serve as a main contact for assigned patients and family members
• conduct follow-up calls after visits
• review care plans and help ensure follow-up appointments are scheduled
• screen for access barriers, distress, and understanding of diagnosis
• refer patients to outside resources
• communicate with the multidisciplinary team and providers
• enroll and maintain patients in management or value-based programs
• support initiatives to improve workflows, data collection, and care quality
• cross-train in scheduling, call center, and registration tasks.
