The Registered Nurse, Wooster Heart Group Phone Triage role is onsite in Wooster, OH. The nurse assesses patient concerns by phone, determines urgency, and coordinates appropriate care and follow-up. The position also provides in-office clinical support as needed to help maintain patient access, safety, and efficient practice operations.

What you'll do

• Assess patients by telephone using clinical judgment and established protocols
• determine urgency and appropriate level of care
• provide advice, education, and follow-up instructions
• manage calls, portal messages, and refill requests
• coordinate with providers and departments
• document interactions in the EMR
• communicate results and recommendations
• escalate complex concerns
• arrange referrals and follow-up
• support in-office care as needed, including rooming patients, vital signs, procedures, medications, phlebotomy, and wound care.
