The Activities Assistant and Transportation Specialist role is onsite in Fort Collins, CO, with a Saturday–Wednesday schedule from 9:00 AM to 5:00 PM. Pay is $17–$21 per hour, depending on experience and qualifications. The role supports residents’ quality of life and care at Storybrook Care and Rehabilitation, a skilled nursing facility. The position assists with resident activities and safely transports residents to appointments, therapy, and community outings. It helps meet residents’ recreational and social needs while supporting safe, respectful transportation and communication with facility staff about resident needs or changes.

What you'll do

• Assist the Activities Director with planning and supporting quality activities
• inspect the facility vehicle and help ensure routine maintenance
• transport residents safely and secure them during trips
• coordinate transportation requests with departments
• observe resident mood and behavior and report concerns or condition changes
• treat residents with dignity and respect
• follow facility policies, safety, hygiene, infection-control, confidentiality, and resident-rights requirements
• assist with resident movement or activities of daily living when needed.
