What you'll do

• Assist with activities of daily living and hygiene
• support wound care, medication reminders, patient monitoring, rooming, vital signs, and basic assessments under RN supervision
• participate in harm reduction education and supply distribution
• collaborate with the interdisciplinary team
• assist with mobile and outreach efforts
• document patient interactions
• follow safety, infection-control, and compliance procedures.
