What you'll do

• Oversee the Social Services Department and preadmission services
• participate in interdisciplinary evaluations and complete PASARR Level I
• coordinate discharge planning and community-based services
• help residents access financial, legal, and community resources
• oversee documentation and departmental quality procedures
• coordinate advance care planning and review advance directives
• address grievances and track trends
• contribute to facility policy and quality initiatives
• promote resident rights and dignity
• report and investigate suspected abuse, neglect, or exploitation
• ensure access to treatment and resources for mental illness and psychosocial difficulties
• manage Social Services office employees.
