What you'll do
- Lead and supervise the Clinical Coordinator, field nurses, and caregiving staff
- oversee client Plans of Care
- conduct assessments, reassessments, supervisory visits, and safety evaluations
- update individualized care plans
- review charts and documentation
- manage incident reports and risk mitigation
- support accreditation and regulatory compliance
- coordinate staff orientation and training
- coordinate care with providers and families
- support urgent patient-care concerns
- assist with referrals, admissions, discharges, and transitions
- and provide skilled nursing care within the Ohio Nurse Practice Act.
