What you'll do

• Assess clinical nutrition needs
• develop and document care plans in the electronic medical record
• implement and evaluate individualized plans using clinical findings, lab results, weight trends, intake, and comorbidities
• provide nutrition education and counseling to residents, patients, families, and staff
• collaborate with interdisciplinary teams
• follow CMS, state, and federal regulations and company standards. Coverage in multiple buildings is based on company needs and is described as optional/as desired.
