What you'll do

• Conduct in-home assessments and sign-up visits
• determine service needs, level of care, and client preferences
• develop individualized service plans
• complete and submit visit documentation
• conduct supervisory visits to evaluate caregiver performance and adherence to care plans
• educate caregivers on safety, infection control, and personal care
• report significant condition changes and assist with plan updates
• liaise among clients, families, caregivers, and office staff
• maintain compliance with state and company RN supervision requirements.
