What you'll do

• Maintain an active patient panel and provide preventive and primary care
• diagnose and manage acute and chronic conditions
• supervise prescribing practices of assigned advanced practice providers
• coordinate care with nursing, care management, behavioral health, and support staff
• assess social determinants of health and address barriers
• document encounters and submit billing documentation within required timeframes
• participate in quality improvement, peer review, and patient safety activities
• participate in a 24-hour on-call rotation approximately four times per year.
