What you'll do

• Oversee patient care services and personnel
• coordinate assignments, referrals, and patient care
• assess patient needs and support individualized care plans
• coordinate with hospitals, physicians, discharge planners, clinicians, patients, caregivers, and internal teams
• train and review employees
• prioritize urgent and high-acuity cases
• support compliance and quality improvement
• coordinate workflow across Lombard and Chicago branches.
