What you'll do
- Perform comprehensive patient assessments and document findings

- educate patients and families on care plans, medications, and home care
- respond to patient concerns
- collaborate with the Director of Nursing about changes in condition
- maintain accurate documentation
- develop, update, and follow individualized care plans
