Registered Nurse (RN) – Home Health & Hospice

The Registered Nurse (RN) – Home Health & Hospice role is an onsite, part-time position based in Livingston, TX, with pay of $38 to $50 per hour. The nurse provides care in patients’ homes and hospice settings as part of the Home Health and Hospice team. The RN delivers skilled, patient-centered care and coordinates with patients, families, physicians, and interdisciplinary colleagues to support clinical outcomes, safety, comfort, and continuity of care. Responsibilities include assessment, care planning, treatment, education, documentation, and assigned on-call participation.

What you'll do

• Conduct initial and ongoing patient assessments
• develop care plans with physicians, patients, and families
• administer ordered medications, IV therapies, wound care, and other skilled nursing procedures
• monitor and report changes in patient condition
• educate patients and families
• coordinate with interdisciplinary care teams
• document in the electronic medical record
• participate in on-call rotation as assigned
• follow applicable policies and regulations
• provide end-of-life care and emotional support in hospice settings.
